Transportation Requests
Please Note: Emails can take from 1 hour to 24 hours for us to receive after you have sent them. We must receive your email before 2:30 p.m. each day, 48 hours before your appointment, not including weekends and holidays. If you have any doubts, please call us with your request.

– Date and time of appointment
– Approximate length of appointment
– Social Security Number of client
– Client’s full name
– Parents full name (If patient/client is under 18 years of age.)
– Client’s full address (place to pickup)
– Client’s phone number
– Client’s email address
– Destination name and full address
– Will the client need to go to a pharmacy after the medical appointment?
– Pharmacy name and address
– Total passengers
– Do you need a handicap van?
– How many car seats or booster seats do you need?
– Special assistance needed and any other information.
